The state of Indiana has specific requirements for reporting on an annual basis. Generally this is submitted at the corporation level. The following is a framework of the required information:

Objective Measures of Student Achievement and Growth
Rigorous Measurements of Effectiveness
Designation in Rating Category
Evaluation Feedback
Evaluation Plan Discussion
Evaluators
Feedback and Remediation Plans
Instruction Delivered by Teachers Rated Ineffective
Attached is the Staff Performance Evaluation Plan Submission Coversheet SY 2019-2020 for reference providing more indepth guidance. The due date is generally in mid-September.
